Leading the Way In Memory Care

We provide a loving and joyful environment where Elders are cared for as family and can experience well-being while living life to its fullest. Through our mindful and compassionate approach, we inspire our residents to live a happy and dignified life. Our philosophy of quality care recognizes meaning, purpose and fulfillment for all.

The Memory Care Residences at Creekstone were specifically designed to care for persons living with memory loss. There are no long corridors, nurse's stations or medication carts at our four residencies. We offer a welcoming environment where residents can thrive in mind body and spirit while being nurtured by the comforts and routines of home and family.

Teepah Snow’s Positive Approach to Care (PAC) training was completed by 17 of our RPH staff, certifying them to now become trainers and/or mentors within all three of the RPH communities.  They learned the PAC philosophy, providing dementia related awareness, knowledge and skills allowing staff to create a more positive and caring environment for our residents who are living with memory loss.  This training was funded by the Ralph C. Wilson, Jr. Foundation.
